New Patients

  • All new patients must complete & sign the forms
  • New patients using insurance must bring the following:
    • Referral from a physician (MUST HAVE IT FOR YOUR FIRST APPT.)
    • Insurance Card(s)
    • VALID ID (Driver's license, State ID, Military ID, Passport etc.)
    • Op-Note (Surgery Note) (if applicable)
    • X-Rays, MRI & CT Scans (if applicable)
    • Primary care physician (PCP) referral, if required by your insurance
  • New patients using workers' compensation insurance must bring the following:
    • Referral from a physician (MUST HAVE IT FOR YOUR FIRST APPT.)
    • Claim Number
    • VALID ID (Driver's license, State ID, Military ID, Passport etc.)
    • Op-Note (Surgery Note) (if applicable)
    • X-Rays, MRI & CT Scans (if applicable)
  • It is the patient's responsibility to confirm that we participate with his/her insurance plan and also to bring a Primary care physician (PCP) referral, if required by your insurance.
  • Please arrive 20 to 25 minutes before your initial appointment to allow time for paperwork.
  • You may arrive on time for your initial appointment if you bring the completely filled and printed forms.

Cancellation Policy

  • $80.00 fee will be charged in the event you are unable to attend your appointment, unless you provide a 24 hours advanced notice (between Monday – Friday between 8am to 5 pm) prior to your appointment.
  • $80.00 fee will be also be charged for late arrivals (10 minutes or more), missed appointments and appointments not cancelled or rescheduled 24 hours (between Monday – Friday between 8am to 5 pm) prior to the scheduled appointment time.

Inclement Weather Policy

You may call or text (703) 242-4263 to check if we are open. Appointments canceled during inclement weather are not subject to cancellation fee.
