WE DO NOT PARTICIPATE WITH MEDICAID, HUMANA, KAISER & HMO PLANS OF AETNA, ANTHEM, CAREFIRST, CIGNA & UNITED HEALTHCARE.
We participate with PPO Plans only (does not apply to Medicare, FEP, GEHA, Tricare & Workman's Comp Plans). PPO plans do not require authorization or reauthorization for therapy. We do not file authorization or reauthorization requests. It is the patient's responsibility to verify with his/her insurance prior to attending therapy.
It is the patient's responsibility to confirm that we participate with your insurance plan and also to bring a Primary care physician (PCP) referral, if required by your insurance.
